Form Builder Question Types

When you create a new form, it starts off with a Contact Form and a Thank You page.

You then have a selection of Questions to choose from and add to your form. You can add as many of these Questions as you want.

There are different types of Questions based on the different types of input they’ll take. For example, a Dropdown Question includes a dropdown list, and a Birthday Question includes a Month, Date, and Year. 

The list below will tell you how to configure each one.

There are Common Settings that you can edit for all Question Types, as well as specific settings based on the type of input the Question takes.

Standard Questions

Birthday

The Birthday question makes it easy for your users to enter their birthday. It includes 3 dropdown menus for MONTH, DAY, and YEAR.

By default it looks like this: 

  • Question is set to “When is your birthday?”
  • Description is set to OFF

You can edit the Question, Description, CTA, and Advanced Options just as you would any other question.

Options

  • “How far back do you want to display?” determines the oldest year people can select.
    • It defaults to 100 years before the current year; i.e., 1922.
    • Users cannot select a year that occurred before the time period you select.
    • If it’s set to 0, the YEAR dropdown will only show the current year. 
  • “Set minimum required age:” determines the most recent year people can select.
    • It defaults to 18 years before the current year; i.e., 2004.
    • Users cannot select a year that occurred after the time period you select
    • If it’s set to 0, there is no minimum, and the YEAR dropdown starts with the current year. 

Possible Error Messages

  • If the user selects a date later than the current date, they get this error message: “Your selected birthday is less than minimum required age.”

CTA Message

The CTA Message is an easy way for you to add a CTA button that starts off your form or takes them away from it. 

It includes a CALL-TO-ACTION, DESCRIPTION, and CTA BUTTON.

By default it looks like this: 

  • Call-to-Action is set to “Welcome! Let’s get this party started.”
  • Description is set to ON and includes ideas on what to tell your users
  • CTA Button is set to NEXT STEP

You can edit the Call-to-Action, Description, and Advanced Options just as you would any other question.

  • There is no Automatic Progress option because the CTA Button will advance the user to the next step or other location.

CTA Button

This CTA Button is different from the CTA Button on other question types.

  • By default the Button Text says “Get Started”, but you can change that to anything up to 22 characters long
  • What should it link to?” determines where the button takes the user
    • Next Step: takes them to the next Question in your list
    • A leadPops Lead Form: lets you select a Lead Form to take them to
    • Outside URL: lets you enter a completely different URL to send them to
  • Font Size can be between 12px and 50px
    • It’s a good idea to use the same Font Size on all your buttons
  • Button Icon is an icon that displays next to your Button Text and is turned OFF by default
    • You have at least 16 icons to choose from
    • You can set the Icon Color using a color picker
    • The Icon Positioning can be on the LEFT or the RIGHT of the Button Text
    • The Icon Size can be between 12px and 50px and you can lock the Icon Size to always match the Font Size.

Dropdown

The Dropdown question makes it easy for you to get your users to select an option from a dropdown list. 

This works best when there are a lot of options to choose from and you only want them to select one, although you can set it to allow multiple selections.

It includes a QUESTION, DESCRIPTION, and Dropdown with OPTIONS.

By default it looks like this: 

  • Question is set to “Enter your question here.”
  • Description is set to OFF
  • There are 3 possible Options in a Dropdown

  • There are additional settings within the Advanced Options section for this question.

Options

The Options are what users pick from in the Dropdown list to answer your question.

  • You have 3 Options configured by default
  • You can edit the text for each Option directly in the list
  • To add a new one, select an existing one, then hit ENTER.
  • You can drag-and-drop to change their list order or delete them as needed
  • To allow users to select multiple options, set “Multiple Selections” to ON
  • If you’re using “Multiple Selections”, you can include “None” as an option

You can also add a Field Label up to 30 characters long to appear in the Dropdown field.

Advanced Settings

  • “Add Other Option” gives users an option labeled “Other” and a text box to enter their answer
  • “Alphabetize” puts the options in alphabetical order
  • “Automatic Progress” moves the user to the next step once they make a selection; it requires less clicks and keeps the user moving
  • “Randomize” changes up the order of options for each new users to remove bias
  • “Required” makes it required for a user to select an option before they continue
  • “Search Mode” gives the user a search field to find their answer

Possible Error Messages

  • If the “Other” option is set, and they don’t select an answer, they get this error message “Please enter your answer.”

Hidden Field

A Hidden Field lets you include data in the Lead Form that your users can’t see or edit, such as a Database Record, to submit with the form answers.

You must include a Field Label, 1-3 words in length.

The Default Value is optional, and so is allowing the field to be set dynamically.

Menu

The Menu question makes it easy for you to get your users to select an option from a group. Users will see all their options right away and can make their selection easily.

This works best when there’s only a few options or you want to let them select multiple options.

It includes a QUESTION, DESCRIPTION, and group of OPTIONS.

By default it looks like this: 

  • Question is set to “Enter your question here.”
  • Description is set to OFF
  • There are 2 possible Options

Options

The Options are what users pick from in the Dropdown list to answer your question.

  • You have include up to 8 Options before it becomes a Dropdown list
  • You can edit the text for each Option directly in the list
  • To add a new one, select an existing one, then hit ENTER.
  • You can drag-and-drop to change their list order or delete them as needed
  • To allow users to select multiple options, set “Multiple Selections” to ON
  • If you’re using “Multiple Selections”, you can include “None” as an option

Advanced Settings

  • “Add Other Option” gives users an option labeled “Other” and a text box to enter their answer
  • “Alphabetize puts the options in alphabetical order
  • “Automatic Progress moves the user to the next step once they make a selection; it requires less clicks and keeps the user moving

Number

The Number question makes it easy for you to get your users to enter a number. It can be a simple digit, a percentage, or a dollar amount. It includes a QUESTION, DESCRIPTION, and NUMBER field.

By default it looks like this: 

  • Question is set to “Enter your question here”
  • Description is set to OFF
  • Options are set to no Minimum or Maximum with no formatting except commas
  • CTA Button is set to “Continue”

Options

Users can only enter digits and decimal points in the Number field.

The Options for your number determine the range you’ll accept.

  • Minimum Number is the lowest number you’ll accept
  • Maximum Number is the highest number you’ll accept

If you add a minimum or maximum setting, consider mentioning that in your Description. 

Advanced Settings

  • “Auto-Cursor Focus” automatically puts the cursor on the Number field so users can start typing their answer in quickly
  • “Auto-Format with Commas” adds commas to the number to indicate 1000s
  • “Format as Currency ($)” adds a USD symbol to the front of the number
  • “Format as Percentage (%)” adds a percentage symbol to the back of the number
  • “Required” makes it required for a user to select an option before they continue

Possible Error Messages

  • If users enter a number lower than your minimum, they get this error message “Input is less than minimum number.”
  • If users enter a number higher than your maximum, they get this error message “Input is more than maximum number.”

Slider

The Slider question is an interactive way to get your users to select a number. It lets them slide left and right on a scale that you create. You can set it to show whole numbers or number ranges.

It includes a QUESTION, SLIDER, and CTA.

By default it looks like this: 

  • Question is set to “Enter your question here.”
  • Slider is set to 20,000 increments of $200K to $2M+
  • CTA is set to Continue

Slider Setup

There are a lot of options to choose from for your Slider.

  • A 1-Puck Slider allows users to select a specific value
  • A 2-Puck Slider allows users to select a minimum and maximum value

Slider Range

The Slider Range determines what range of numbers users can select from

  • The selected value can appear as dollars ($), percentages (%), or digits (#)
  • The selected value must have a Minimum and Maximum number
  • “By: Subranges” means the selected value will appear as a range (for example, a Subrange of 20 and a maximum of 100 would have options like 40-59 and 60-79. You can include multiple subranges for higher and lower numbers like a Subrange of 20 up to 100, then a Subrange of 100 up to 500.
  • “By: Increments” means the selected value will appear as a single digit (for example, an Increment of 20 and a maximum of 100 would have options like 40, 60, 80, etc.
  • “Starting Number Ends with: 0” will give the user options like 40-59, 60-79, and 80 because they start with 0.
  • “Starting Number Ends with: 1” will give the user options like 41-60, 61-80, and 81 because they start with 1.
  • If you select a 2-Puck Slider, users can only select “By: Increment” since they’ll be creating their own range by selecting two different increment values.

Slider Labels

The Slider Labels are the copy that displays on the left and right side of the Slider Range as well as the Selected Value that displays if a user selects the minimum or maximum number. 

  • The labels should line up with the Minimum and Maximum numbers you selected for the Slider Range.
  • They don’t have to be exact duplicates, though. Your Maximum could be $100, but your Right Label and Slider Value could show $100+ to indicate more than $100.
  • If you turn the Slider Labels OFF, the Minimum and Maximum you selected for the Slider Range will display.

Slider Starting Point

The Slider Starting Point is the default value that’s selected when users first see the Question.

  • If you turn the Slider Starting Point OFF, the Selected Value will default to the lowestpossible value.

Advanced Settings

  • “Show Billions as B” simplifies the field value as B to indicate billions
  • “Show Millions as M” simplifies the field value as M to indicate millions
  • “Show Thousands as K” simplifies the field value as K to indicate thousands

You can only select one of the options above at a time.

Text Field

The Text Field question makes it easy for you to get your users to enter a text answer. It can be a simple digit, a percentage, or a dollar amount. It includes a QUESTION, DESCRIPTION, and NUMBER field.

By default it looks like this: 

  • Question is set to “Enter your question here”
  • Description is set to OFF
  • Field Label is set to “ENTER YOUR ANSWER”
  • CTA Button is set to “Continue”

Options to Consider

  • SHORT TEXT INPUT is meant for answers 30 characters or less. It displays as a single line of text entry.
  • LONG TEXT INPUT is meant for answers with more than 30 characters. It displays as a multi-line text box.
  • There’s no maximum character limit for each input option.
  • The Field Label can be up to 30 characters. You can turn it OFF to remove it.

Advanced Settings

  • “Auto-Cursor Focus” automatically puts the cursor on the Text field so users can start typing their answer in quickly
  • “Required” makes it required for a user to enter at least some text before they continue

Zip Code

The Zip Code question makes it easy for you to get your users to enter their zip code. 

It includes a QUESTION, DESCRIPTION, and NUMBER or TEXT field that searches for the correct location.

By default it looks like this: 

  • Question is set to “Enter your question here”
  • Description is set to OFF
  • Field Label is set to “IP CODE”
  • CTA Button is set to “Continue”

  • There are additional settings within the Advanced Options section for this question.

Options to Consider

  • ZIP CODE ONLY only allows users to enter a 5-digit zip code
  • CITY OR ZIP CODE allows users to enter a 5-digit zip code or city name and then select the correct location from a list.
  • There’s no maximum character limit for each input option.
  • The Field Label can be up to 30 characters. You can turn it OFF to remove it.

Advanced Settings

  • “Required” makes it required for a user to enter an accepted value before they continue
  • “Automatic Progress” is only for CITY OR ZIP CODE and moves the user to the next step once they make a selection; it requires less clicks and keeps the user moving
  • “Require U.S. Zip Code” is only for ZIP CODE and checks to make sure the zip code exists in the US before users can submit it
  • “Require Canadian Postal Code” is only for ZIP CODE and checks to make sure the postal code exists in Canada before users can submit it

Car Make and Model

(Appears under the More Options list of Questions)

The Car Make and Model question makes it easy for you to get your users to enter their Car Make and Model using Dropdown lists. 

It’s a 2-Step Question (one for Car Make, the other for Car Model). Each includes a QUESTION, DESCRIPTION, Dropdown with OPTIONS, and CTA.

By default it looks like this: 

  • Question is set to “What make is your Car?” and “What model is your Car?”
  • Description is set to OFF for both
  • The Field Label is set to “CAR MAKE” and “CAR MODEL”
  • The CTA Button is set to “Continue” for both

Image Answers

(Appears under the More Options list of Questions)

The Image Answers questions makes it easy for you to get your users select the answers based on the visual representation of the options.

It includes a QUESTION, DESCRIPTION, and ADD ANSWER button.

By default it looks like this: 

  • Question is set to “What type of place are you looking for?”
  • Description is set to OFF

 

You can edit the Question, Description, CTA, and Advanced Options just as you would any other question.

Options

The options are what the user pick to answer your question.

  • You can customize how the options appear by clicking the ADD ANSWER button and uploading an image from your computer, browse image gallery, or browse icons.
  • You can also set the Label, and Image Alt for each Image option.
  • You can customize them on how they will appear in the question.
  • You can set the Background Color using a color picker
  • You can set the Text Color for the label using the color picker
  • The Border Radius can be between 0px and 12px
  • To enlarge the Image Answer options, set Supersize Image to ON
  • To show the labels, set the Show Labels to ON.
  • To allow users to select multiple options, set “Multiple Selections” to ON

Address

(Appears under the More Options list of Questions)

The Address questions makes it easy for you to get your users provide address information, and have it validated against Google maps.

It includes a QUESTION, DESCRIPTION, and GOOGLE MAP VERIFICATION STEP.

By default it looks like this: 

  • Question is set to “Where do you live?” for the first step and "Does this general map listing look correct?" for the second step.
  • Description is set to OFF

 

You can edit the Question, Description, and CTA just as you would any other question.

Settings

  • “Required” makes it required for a user to enter at least some text before they continue
  • "Show Unit #" is added in the Address field as additional detail.
  • "U.S. Only" makes the address input restricted to U.S based locations only.
  • "Specify U.S. State(s)" makes the address input restricted to the selected U.S. state only.
  • “Auto-Cursor Focus” automatically puts the cursor on the Number field so users can start typing their answer in quickly.
  • "Google Map Verification Step" enables the second step, and validates the address entered from the first step.
    • For the first step, the "Field Label" can be set. By default, it has a value of "TYPE IN YOUR ADDRESS".
    • For the second step, the "Button Labels" can be set. By default, the two labels are displayed as "Text to confirm address" and "Text to decline address".

Contact Form

(Added as the last Question to all Lead Forms by default)

The Contact question is an easy way for you to get the user’s essential contact info: Name, Email Address, and Phone Number.

You can set it up as a 1 Step, 2 Step, or 3 Step process:

  • 1 Step means you ask the user for Name, Email, and Phone all in 1 Step
  • 2 Step means you ask the user for Name and Email in Step 1, then Phone in Step 2
  • 3 Step means you ask the user for Name, then Email, then Phone in 3 different steps

The tab selected when you save determines the input process that will display.

Each Step includes a QUESTION, DESCRIPTION, INPUT TYPES, and CTA.

By default it looks like this: 

  • 1 Step process 
  • Question is set to “What’s the best way to reach you?”
  • Description is set to OFF
  • The Contact Form Setup is set to “FULL NAME”, then “EMAIL ADDRESS”, then “PHONE NUMBER”
  • The CTA Button is set to “Continue”

No matter what Step Process you select, you can edit the Question, Description, CTA, and Advanced Options just as you would any other question.

  • There are additional settings within the Advanced Options section for this question.

Contact Form Setup

  • Depending on the Step Process you selected, there could be up to 3 Inputs
  • For the 1 Step and 2 Step process, you can click Organize to change the order of Inputs for Steps with more than 1 Input
  • For Input Type: Name, you can request only the First Name, the First Name and Last Name in different fields, or the Full Name (first and last) in one field
  • For Input Type: Phone, you can click Settings to require a Country Code or set it to auto-format based on the Country Code
  • For all 3 Input Types (Name, Email, Phone), the Field Name can be up to 30 characters

Advanced Settings

  • “Required” lets you select which Inputs are required on each step for the user to continue
  • “Auto-Cursor Focus” automatically puts the cursor on the Text field of the first Input so users can start typing their answer in quickly

Common Question Settings

These are the settings that appear across every Question Type. The same information and options apply for each one. 

Question

This is basically your heading. It’s where you enter the question you want to ask.

By default, the text displays in Open Sans, 28pt font. You want it to stand out.

But… you also have a whole content editor where you can insert and format different content however you want. That includes:

  • Images
  • Videos
  • Emoticons
  • Click-to-Call Numbers
  • Font Type/Size/Style

Description

This is like a subheading. It’s where you add additional information to further instruct, motivate, and encourage users to fill out your question. 

It’s optional, so you can turn it OFF if you don’t want to use it.

By default, the text displays in Open Sans, 18pt font.

But… you also have the same content editor as you did for the Question, where you can insert and format different content however you want. That includes:

  • Images
  • Videos
  • Emoticons
  • Click-to-Call Numbers
  • Font Type/Size/Style

CTA Button

This is the button your users click to go to the next question.

Most Question Types will include a CTA Button by default, but if you turn ON the Automatic Progress option for a Question, you can choose to turn OFF the CTA Button.

  • By default the Button Text says “Continue”, but you can change that to anything up to 22 characters long
  • Font Size can be between 12px and 50px
    • It’s a good idea to use the same Font Size on all your buttons
  • Button Icon is an icon that displays next to your Button Text and is turned OFF by default
    • You have at least 16 icons to choose from
    • You can set the Icon Color using a color picker
    • The Icon Positioning can be on the LEFT or RIGHT of the Button Text
    • The Icon Size can be between 12px and 50px.
      You can lock the Icon Size to match the Font Size.
  • You can Hide the CTA Button the question is answered as a way to require them to answer it

Advanced Options

Security Message

The Security Message is a “Privacy & Security Guaranteed” badge that displays with a lock icon below the Question Input and CTA Button. It’s a way to promise your potential leads that the answers they submit are secure with you.

You can’t edit the badge, but if you want to create something similar with your own message, see the Additional Content option.

Additional Content

Additional Content is your way to add almost anything you want to the bottom of the question. It has all the same content and formatting options as the Question and Description.

Be careful, though. You don’t want to add too much and distract people from filling out the question. It is a good place, however, to add endorsements or reviews and other content you want to differ across each question.

It’s a lot like Extra Content, except the Additional Content for a question only displays for that question.

Field Details

The only Field Details setting has to do with the Unique Variable Name.

This is auto-generated for each Question Type and helps us store the values each person inputs for that question.

We recommend you do not edit the Unique Variable Name unless you have a specific integration reason. Contact us if you have any questions.
